For our team, communication is an open discussion about the diagnosis, prognosis, treatment options, and informed decision making that goes along with the patient’s specific case. Proper communication throughout the channels itself can be a significant relief to patients and their families. Simply having someone to coordinate can remove at least one challenging step for families.

Coordination

Coordination is ensuring that the patient, family, caregivers, and professional understand the patient’s goals and objectives. Based on these objectives, care is able to be provided to orient toward these goals. By coordinating these conversations, palliative care can allow patients and their families to focus on what truly matter most to them, not trying to coordinate details.
